UNIVERSITY HR BENEFITS PRESENTATION - 4/4/2006
NEW EMPLOYEES Your benefits are effective upon your first day of employment, even though you will not be making your benefit selections until a few weeks after you have been employed. Generally it takes about 4 weeks from the time of your employment to receive your health/dental application forms in the mail. If you or a family member need to see a physician or dentist before you have received and returned these forms, you will still be eligible for benefits, based upon the plan you chose. However, you will need to contact the OSU Benefits Office to discuss what kind of "proof" they can provide to your physician verifying that you are covered by OSU benefits. Once you have submitted your application forms to the OSU Benefits Office, it will take about 2 weeks to receive your health/dental cards. If more than 2 weeks have passed and you still have not received your cards, please call the number below for assistance: OSU Benefits Office or locally:
MAKING CHANGES TO YOUR BENEFIT SELECTIONS Changes to your existing health, dental, or vision benefits can only be made during an Open Enrollment period, which occurs every April. Changes can also be made any time during the year if you have a 'qualifying event.' A qualifying event would be marriage, divorce, birth of a baby, or loss of coverage on a spouse's insurance plan. If you have a qualifying event, please click here to print the forms you will need to inform us of your family change: After your forms are returned to the Extension Payroll Office, the OSU Benefits Office will send you new forms to fill out so your benefit information can be updated.
